Supportive Choices provides tailored NDIS Support Coordination, Plan Management, and Therapeutic Supports to help you maximise your plan and achieve your goals. 

We strive to build your confidence and equip you with the skills to coordinate your supports independently.

Support Coordination Levels:

  • Support Connection – Linking you with the right services and resources.
  • Support Coordination – Building skills to use your plan effectively and live independently.
  • Specialist Support Coordination – Intensive help for complex needs.

Plan Management services include payment processing, budget tracking, provider choice, and guidance to improve your financial and self-management skills.

Therapeutic Supports help NDIS participants improve independence, mobility, self-care, social interactions, and community involvement.
Funded mainly under the Capacity Building Supports budget, they cover a range of therapies and professional services.

Therapeutic Support

Support Coordination

Support coordination helps you effectively use your NDIS plan to reach your goals by connecting you with providers, community resources, and government services. It also builds your confidence and skills in managing your supports. A support coordinator or specialist support coordinator provides this service.

There are three levels of support coordination in an NDIS plan:

  1. Support Connection – Helps you connect with informal, community, and funded supports, enabling you to maximize your plan and pursue your goals.

  2. Support Coordination – Provides assistance in understanding and using your plan, helping you build the skills to manage services, maintain relationships, and increase independence.

  3. Specialist Support Coordination – A higher level of support for individuals with complex needs, helping them navigate challenges and ensure consistent service delivery.

Plan Management

Plan management is a service under the NDIS that helps participants manage their funding. It supports participants by:

  • Overseeing and tracking their budget

  • Managing NDIS claims and paying providers for services

  • Providing regular financial updates, including alerts about under or over-utilisation of funds

  • Giving participants more control over how their plan is implemented

Additionally, plan managers can offer advice to help participants better understand and use their NDIS funding, enhancing their financial knowledge and capacity.

Therapeutic Supports

 

Our Therapeutic Support services are delivered by registered allied health providers, one of the largest groups of registered providers.

They include services like nursing for complex needs, counselling, therapy assessments, assistive technology advice, exercise programs, and diet planning. 

Therapeutic supports fall into three main Capacity Building categories:

  1. Improved Daily Living – Therapies to boost independence, confidence, and community participation.

  2. Improved Health and Wellbeing – Supports from dietitians and trainers to maintain and improve health.

  3. Improved Relationships – Social skills development and behavioural interventions.
