Dedicated to Helping You Achieve Your Goals Listed In Your NDIS Plan

At Supportive Choices Pty Ltd, we specialise in providing comprehensive support coordination services to ensure you maximise the benefits of your NDIS plan.

Our tailored services help you understand and utilise your plan effectively to achieve your goals.

Our team connects you with suitable NDIS providers, community services, and mainstream and other government services, thus broadening your network of support.

We strive to build your confidence and equip you with the skills to coordinate your supports independently.

We cater to three levels of support coordination, each specifically designed to cater to your unique needs.

Our Support Connection services are designed to enhance your ability to connect with various support resources, helping you get the most out of your plan.

Our Support Coordination services focus on assisting you in building the skills needed to understand and use your plan effectively. A coordinator will guide you in ensuring you get the support you need to live independently and feel included in your community.

We also offer Specialist Support Coordination for those with more complex situations, providing them with higher levels of support to manage challenges in their support environment.

Beyond support coordination, we offer exemplary NDIS Plan Management services.

Our plan managers are dedicated to helping you improve your financial and plan management skills, understand how to self-manage your plan, facilitate payments to providers, increase your choice of providers, and provide comprehensive NDIS plan budget reports for better budget oversight.

Our Supports

Our Therapeutic Support services are delivered by registered allied health providers, one of the largest groups of registered providers. They provide evidence, assessments, and reports that inform access and planning decisions under the NDIS Act.

They can refer you to information about NDIS access, provide supporting evidence for an access request, and provide reports or assessments that describe the extent of the functional impact of the disability.
